Profession: RN – Case Manager
Location: 1975 Research Pkwy, Ste 300, Colorado Springs, CO 80920
Weekly Pay Package:
• Tax-Free Stipend: $1,162.00
• Straight Pay (Taxable): $1,280.00
• Gross Weekly Total: $2,442.00
Schedule:
• Hours/Day: 8
• Hours/Week: 40
• Shift: Day, 8a–5p (No on-call, weekends, or OT)
• Travel: Required within Colorado Springs area
License & Requirements:
• Active CO RN or Multi-State License
• Reliable transportation and car insurance required
• Preferred: HCHB experience (Hospice Care)
Primary Responsibilities:
• Conduct comprehensive assessments for high-risk members
• Collaborate with PCPs to implement individualized care plans
• Identify and reassess cost-effective, appropriate levels of care
• Coordinate non-medical support (e.g., housing, transportation)
• Manage care transitions with physicians, social workers, and discharge planners
Engage specialty resources to support optimal outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Promote health and prevent illness through proactive interventions
• Act as patient advocate, ensuring privacy and confidentiality
Qualifications:
• Associate’s or Bachelor’s Degree in Nursing or related field
• Current, unrestricted RN license
• Case Management certification
• Strong analytical skills for complex medical decision-making
